Kathy Leoffler started our diabetic program in 2008. This program includes group visits, individual patient visits, peer coaching, and education.

In 2010, Kathy Leoffler provided education for 88 of our patients concerning diabetic healthcare, diet, sugar levels, and tracked their A1C levels. Out of the 88 patients only 5 visited the ER that year for diabetic related issues.

In October 2011, Fayette Care Clinic received a grant from American Medical Association Foundation for $10,000 to enhance their diabetic programs. Thank you American Medical Association Foundation.
